Background: "Curvi-linearity" observed in the descending segment of the forced expiratory flow-volume curve is a hallmark for airflow obstruction. Currently, there is no available spirometric index that directly quantifies the degree of curvature. We propose a "curvature" index, kmax , and we examined a sample of spirometric data to explore the relationship between kmax and FEV1. The Model: A simple quadratic function boQ + b1QV + b2V 1 ; is fit to the descending phase of a flowvolume curve: Q flow rate, V volume and bo, b1, b2 are unknown coefficients to be estimated from a goodness of fit analysis of the patient's flow-volume data. The relationship between kmax and FEV1 kmax 2.015 e -1.245FEV1. We observed that when FEV1 2.0 L, the index kmax shows a wide range of variation. Conclusions: kmax relates, exponentially, to FEV1. This curvature index may be more sensitive to the severity of disease and, therefore, potentially useful in diagnosis and longitudinal monitoring. The central assessors in anaesthesia reviewed the cases of all the women in this report identied as having received an anaesthetic for this triennium, some 142 cases. In looking at the individual cases the assessors were struck by the high standards of anaesthetic care generally provided, sometimes in difcult circumstances. In most cases the standard of record keeping was also high. However, there are areas of concern about the consistency of anaesthetic services between units. Reviewing the records showed that in some places the quality and quantity of anaesthesia service does not meet the declared standards of the relevant professional bodies. In deciding the likely cause of death, the evidence had to be weighed up without the benet of questioning all those involved in a patient's care. Much reliance is placed on the local assessors and their ability to make discreet enquiries. Sometimes, even after coroner's inquests, the cause of death was unclear. It was therefore a matter of judgement assigning a death as being a direct result of anaesthesia, or whether substandard care contributed to the demise. Key points of the cases illustrated here result from the central assessors' judgement, bearing in mind that the purpose of the exercise is to learn from errors and improve patient care in the future. The challenges presented to the obstetric anaesthetist are increasing in number, complexity and severity. Many sick.

To our knowledge, this is the first prospective study to examine registered nurse staffing and the risk of endemic bloodstream infection in infants in the NICU. We found low birth weight, peripherally inserted central catheters, and total parenteral nutrition to be significantly associated with bloodstream infection. These findings are consistent with previous research on infants in the NICU.17, 19, 27-33 After controlling for infants' intrinsic and extrinsic risk factors, we found that the number of hours of care provided by registered nurses was significantly associated with the risk of bloodstream infection in one of our study NICUs. This finding is consistent with other studies on nurse staffing. Nursing workload has been associated with higher rates of infection34-36 and increased mortality.37, 38 The use of pooled or temporary nurses has been significantly associated with infection, 8, 34, 39, possibly because of lapses in aseptic technique by those unfamiliar with the routine care policies of the nursing unit. In the NICU, it has been reported that bloodstream infection is associated with understaffing, overcrowding, and the moving of infants between rooms.8, 9, 11 Furthermore, understaffing has been linked to several infectious outbreaks8, 11 and the transmission of antibiotic-resistant organisms among premature infants.8, 10 Our findings suggest that increasing registered nurse staffing by 1 full-time equivalent could possibly reduce the risk of bloodstream infection by 11% in NICU 2. The fact that the number of registered nurse hours was not associated with bloodstream infection in NICU 1 HR, 1.53; 95% CI, 0.39-6.07 ; may have several explanations. When compared with the infants in NICU 2, those in NICU 1 were significantly smaller and more acutely ill, with more invasive devices, which could outweigh the effect of registered nurse staffing. In addition, the infants in NICU 1 had consistently fewer registered nurse hours of care per day than those in NICU 2. This lack of variation in registered nurse hours in NICU 1 may have made it more difficult to detect an association between registered nurse hours and bloodstream infection in these infants.
